When PDE5 is inhibited, cGMP is increased, which relaxes smooth muscles and allows more blood flow to the penis. The increased blood flow improves erections and helps maintain them longer.

This condition is caused by increased blood pressure within the arteries of the lungs, which can lead to symptoms like shortness of breath, chest pain, and fatigue. The commercially available version of tadalafil was also indicated for use in treating benign prostatic hyperplasia (BPH), which is an enlarged prostate.
